Online forms

The Undergraduate Advising Office is making an effort to "Go Green" by providing some of our most commonly used forms online.

• Declaration Forms
  
  Students must complete their major/concentration declaration form by the end of their sophomore year. Minor or Secondary Concentration declaration forms should be completed by their junior year. Forms can be accessed online, but students are encouraged to meet with an academic advisor to discuss their chosen program(s) of interest.
  
  Major Declaration Form - Bachelor of Arts
   
  Major Declaration Form - Bachelor of Science
   
  Concentration Declaration Form - College of Business
   
  Minor Declaration Form
   
  Secondary Concentration Declaration Form - Arts & Sciences
   
• Grade Replacement Form
  
  Students complete this form when they plan to re-take a course to improve their grade. The form must be completed during the first week of the semester when the student is retaking the course.
  
  Students can grade replace a total of four times throughout their time at Bryant. Each course can only be grade replaced once.
   
• Transfer Credit Request Form
  
  This form is used when a student wants to take a course at another institution, during a summer or winter session.
  
  Students can take courses at a community college until they have reached 60 total credits. At that point, students must take courses at a four-year institution. The last ten courses must be taken at Bryant University.
