ALL HEALTH REQUIREMENTS DUE

 AUGUST 1, 2012

RHODE ISLAND STATE REQUIRED IMMUNIZATIONS:

1. TETANUS-DIPTHERIA  -  within past 10 years; Tdap (preferred booster) or Td
    
2. MMR (Measles, Mumps, Rubella)  -  2 doses required at least 28 days apart; dose 1 given on or after first birthday                                                                                     
3. HEPATITIS B  -  3 doses required                                                                              
4. VARICELLA (Chicken Pox) - 2 doses required or history of disease

Recommended Immunizations:

1. MENNINGOCOCCAL (Meningitis) - 2 dose series (if dose 1 given before age 16)
    
2. GARDASIL (HPV) - 3 dose series
    
3. HEPATITIS A - 2 dose series                                                                                     
4. INFLUENZA                                                                                                             
5. TUBERCULOSIS SCREENING (may be required in certain states and for those in high-risk groups)
    

BRYANT HEALTH REQUIREMENTS:

1. PERSONAL AND HEALTH HISTORY INFORMATION (forms found online at my.bryant.edu)
    
2. PHYSICAL EXAM RESULTS (within one year prior of Bryant entry)
    
3. PROOF OF HEALTH INSURANCE (copy of front and back of health insurance card)
